A study shows a substantial progress in PrEP strategies in Latin America and the Caribbean, where a growing number of countries have initiated to deliver PrEP to people at enhanced risk of attaining HIV. The report also signifies the openings that persist in the process and provides references to progress PrEP policies and enter in the region.

In Latin America and the Caribbean, the occurrence of new HIV cases has only reduced by 1% since 2010. Additionally, the HIV epidemic is focused among men who have sex with men and transgender women, which have an HIV frequency of over 10% and 20%, respectively. PrEP is hence immediately required for these populations.

Luke Murphy from the University of California Los Angeles and colleagues from Peru and Canada studied the PrEP policy model throughout Latin America and the Caribbean. Their goal was to better classify application gaps and prospects to expand admission to PrEP in the district. They revised and controlled information on the state of approval by national controllers of PrEP – a crucial step to further increasing admission to it – and also associate PrEP suggesting guidelines and delivery approaches in the region.

The team inspected many causes, including national guidelines, media reports, press releases, non-governmental administrative reports, and peer-reviewed scientific articles. To be qualified for the study, these sources had to discourse the consent of or access to PrEP, its accessibility methods, clinical procedures or cost for Latin America and Caribbean countries. The investigate did not include contacting experts in each country, such as government administrators, doctors and activists.

Of the thirty-three countries in the region, twenty-two (67%) have procedures approving daily oral PrEP for HIV deterrence with branded “Truvada”, fifteen having also recorded the usage of generic tenofovir/emtricitabine. Truvadais accepted in 11 of 12 countries in South America, 5 of 8 countries in Central America and Mexico, and 6 of 13 countries in the Caribbean. These figures are created on countries about which material were obtainable. For most other countries, no data was found, rather than it being established that PrEP had not been accepted.

The legal setting in the Caribbean appears to be an impairment to PrEP programmers. Of 5 countries which outlaw homosexuality, all but one has not accepted PrEP. Of 4 countries that concluded criminalization in the last 5 years, all but one has no PrEP. Lastly, all 4 countries with no current history of illegalisation of homosexuality have PrEP available.

No countries have permitted cabotegravir-based injectable PrEP. However, Brazil had presented an application at the time of the appraisal, and since then Peru has done the same.

Modes of admission to PrEP are quite flexible. Thirteen of the countries accepting PrEP deliver it using their public health systems. In the alternative cases, PrEP can be attained for free (although sometimes people may have to pay for follow-up tests) through non-governmental organisations, government-led execution programs or clinical trials. Unfortunately, in certain countries such as Suriname and Costa Rica, PrEP can only be acquired at pharmacies or private clinics.

The investigation also found that it is frequently problematic to distinguish between endorsements for branded or for generic oral PrEP. This is vital because generics reduce the cost of PrEP, therefore enable its scale-up. It was also found that in some countries not having officially accepted PrEP, it may be that the medication, sanctioned as HIV treatment, is similarly used off-label for PrEP.

Fourteen countries have medical procedures for PrEP. Prominently, these clearly summaries precise target inhabitants, including transgender women, men who have sex with men, sex workers and serodiscordant couples.
